Job summary

BDCFT is looking for enthusiastic and innovative Band 6 Mental Health Liaison Nurse to be an active part of a team of trained and competent professionals in liaison Mental Health, who deliver a timely and compassionate assessment to patients in care homes.

The successful applicants will need to be an excellent motivated practitioner, able to work within Care home environments, respecting their views and methods, while simultaneously promoting Mental Health and collaborative working. In addition, close relationships and partnership working will be essential with other key stake holders, General Practitioners, Care Home managers and above all the service users and their families.

Other key areas of work that the applicant will need to focus on are, Deprescribing of Anti-psychotic medications for patients in care homes, working closely with Pharmacy Technician and Advanced Nurse Practitioner to work in line with the antipsychotic guidance and deliver for the trust a reduction in antipsychotic use for the region.

The role will be toeffectively implement all the above, maintaining Trust values and a high enthusiasm for quality service delivery.

Main duties of the job

  1. Job Purpose:

To provide a high standard of nursing care for service users within the Care Home Liaison in Bradford, Airedale and Craven

The Mental Health Nurse will visit the care homes and review all patients that are on an anti-psychotic medication, carry out through assessments and discussions with the MDT and look to titrate off antipsychotic medication while supporting the patient and care home.
