Job summary

Bradford City And North (BCAN) Community Mental Health Team for Older People has a rare opportunity for a part time Care Coordinator. Based in the busy city of Bradford we cover the City and North areas of the city.

The multidisciplinary teams comprises of Nurses, Occupational Therapists, Support workers, physiotherapist, assistant psychologists, Psychologists and Doctors. We are committed to the excellent community care of older people with a breadth of mental health problems and those with dementia.

You will be supported in your work with regular caseload management and management supervision. You will be expected and encouraged to engage in clinical supervision. Peer supervision is part of our core daily activity.

Main duties of the job

To assist in the continued delivery and development of high community mental health services, maintaining standards of excellence and ensuring that service delivery meets the needs of patients. The post holder will care co ordinate a caseload of clients and support the various pathways within the CMHT.

The post holder will assist the Team Leader in the effective and efficient management of junior staff members and will be expected to deputise for the team leader in their absence.
