Job summary

This is an exciting and rare opportunity for an experienced Occupational Therapist to join the Intensive Home Treat Team based in ACMH - IHTT Airedale

We are looking for someone to provide a high standard of care for service users within the Acute Community Services in Bradford.

You will be involved in the development of an innovative and tailored Occupational Therapy services to support adults, including older age adults, within an acute mental health setting.

As an Occupational therapist you will be providing clinical risk and mental health assessments, formulations and short-term contingency planning for people presenting in mental health crisis as well as Occupational Therapy assessments. Working within the community, you will provide a range of face-to-face brief, solution focused interventions empowering service users, carers and their families in decision making and signpost to a range of health and social care resources for support.

Main duties of the job

  • Responsibility for the development, planning and implementation of care and treatment interventions for individual patients within the Acute Community Services, including, older adult service users.
  • Act as the Clinical Practitioner within a shift to ensure that the care of patients is of the highest standard.
  • The post holder is responsible for the assessment of care needs, the development of programmes of care, the implementation and evaluation of these programmes.
  • Support the Team Manager and Senior Practitioner to meet quality standards, implement policy & procedure.
  • Manage a clinical caseload from the Acute Mental Health Adult Care Service under the supervision of a Senior Occupational Therapist.
  • Formulate and deliver interventions to meet functional, occupational, & environmental needs in negotiation with the Service User and in liaison with Care Navigator, Carers and relevant others,
  • Work within the occupational therapy process utilising specialist standardised assessments where appropriate.
  • Carry out holistic assessments, individual or group interventions in the most appropriate environmental setting e.g. hospital, community, home, workplace to facilitate seamless continuation of care.
  • Support recovery, social inclusion, maintenance or development of roles and responsibilities within the domains of self care productivity or leisure. Promote and maximise potential for independence.
