Assessment Team Practitioner
This post is an exciting opportunity to work within the Assessment Team , CMHT
The BDCFT (Bradford and District Care NHS Foundation Trust) Community Mental Health Assessment Team (CMH AT) offer a multi-disciplinary, pro-active, equality based and comprehensive triage and assessment service to residents of Bradford, Airedale, Wharfedale and Craven who are registered with a GP within the BDCFT area, and who may be experiencing severe and enduring mental health issues, have complex needs, and may need specialist or other interventions to support their needs.

Main duties of the job
The Post Holder will be responsible for the triage/assessment of routine referrals to the assessment team, signposting to other agencies when appropriate and to the wider CMHT.They will hold a short term caseload and be responsible for the assessment of care needs and the development, implementation and evaluation of programmes of care.
The post holder will provide high standards of skilled mental health care to individuals and their families. The role will include active participation in the service planning and development of the nursing and multi-disciplinary teams.
The post holder will provide clinical supervision and support to junior membersof the team and actively promoting the values and policies of the service and the Trust.
The Community Mental Health Nurse will be an active member of the multi disciplinary team and will work in close co-operation with secondary and primary care based professionals, other community agencies such as Housing, Probation, Police, Children Schools & Families and other Voluntary and Third Sector Agencies.

Managing Self-Participate in regular supervision.-Attend all mandatory training.-Participate annually identifying, developing and agreeing your own development plan with your line manager using the Trust ROAD and KSF process.-Comply with all Trust policies, procedures and protocols.-Pay regard to materials and equipment.-Carry out duties with due regard to the Trusts Equal Opportunity Policy.-Seek advice and support from Team Leader whenever necessary.
