Bradford District Care NHS Foundation Trust's (BDCFT's) Patient Safety, Compliance and Risk function sits within the Directorate of Nursing and is comprised of distinct but inter-related teams:

  • Patient Safety

The Patient Safety team is required to set the strategic direction for patient safety, to deliver on reactive and proactive aspects of patient safety improvement work and to deliver education and training to enable and support front line staff to keep patients and staff safer.

Main duties of the job

The job purpose for the Patient Safety Investigator is to lead Patient Safety Incident Response and Complaints investigations and reviews within the Trust. The key objective of the investigation is to identify underlying system factors that contributed to an incident, complaint or patient safety event. These findings are then used to identify effective, sustainable improvements by combining learning across multiple patient safety incident investigations and other responses into a similar incident type.

The Investigator will work in line with Being Open and Duty of Candour principles, ensuring appropriate investigation/review methodology and systems-based approach to support the Trust's patient safety improvement work. This includes developing a culture of continuous learning and improvement and one where staff feel psychologically safe to raise patient safety concerns; by ensuring responses to incidents and complaints are compassionate, effective, fair and consistent. The postholder will also support the process of learning from incidents, complaints and audit outcomes and ensure these inform organisation wide improvement programmes and learning and development activities.

The post holder is accountable to the Senior Patient Safety Manager and will be required to deputise for them.

The postholder will be required to:

  • Undertake Patient Safety Incident Response and Complaints investigations and reviews as per NHS Frameworks and regulations.
  • Lead and coordinate investigations and other types of incident response and provide high quality, comprehensive, concise, and unbiased reports within the allocated timescale.
  • Engage and collaborate with internal staff involved in the incident/complaint, staff who are experts in their field and with external staff from other agencies, using a wide range of investigative methodologies and approaches.
  • Ensure compassion, empathy and support are demonstrated throughout all investigations for service users, carers, staff, and colleagues.
  • Use relevant documentation to analyse findings e.g. policy, protocol, training records etc, to use the data collected to complete a comprehensive report, using a systems based approach
  • Ensure the findings and recommendations of the report are discussed and agreed with the responsible managers prior to submitting the report for approval.
  • Ensure the investigation/response is conducted with consideration of the following:

  • Undertake external investigations as appropriate.
  • Work collaboratively with HM Coroners Office and relevant internal/external legal services to ensure that comprehensive, high quality information is submitted to the HM Coroner within expected timeframes
  • Liaise with HM Coroners departments and attend HM Coroners Inquests as directed to provide evidence on the findings of investigations.
  • Work with clinical and operational colleagues
  • Encourage a philosophy of continuous improvement and learning from incidents amongst staff and colleagues.
  • Deputise for the Senior Patient Safety Manager in their absence, including occasionally chairing meetings and forums as required in line with agreed terms of reference.
  • Meet with service user/family/carer in line with the Being Open policy, signpost them to other services as appropriate, including them as part of the investigation process.
  • Keep family engaged and up to date on the progress of the investigation and sharing the findings of the investigation as part of the Being Open Process
  • Ensure staff are aware of how to access support.
  • Contribute to the management of the process for ensuring themes and trends from all Patient Safety Incidents and Complaints are identified and triangulated to Trust, Service and Directorate work programmes and occurrence and prevalence of incidents is monitored over time for evidence of change and improvement.
  • Engage in a system for providing early warnings of emerging incident clusters, themes, issues which may indicate an ongoing risk for patients, service users or staff and bring these to the attention of the Deputy Director of Nursing and Specialist Services and the Deputy Chief Executive
  • Share the findings of investigations and reviews with relevant parties and stakeholders.
  • Contribute to sharing learning to support the Patient Safety and Learning Group agenda.
  • Attend Quality and Governance meetings to share findings.
  • Contribute to sharing learning from patient safety incidents, complaints, and audit outcomes.
  • Contribute to providing information to the National Confidential Inquiry into Suicide and Safety in Mental Health
  • Analyse information from external sources to ensure the Trust learns from other reports and agencies.
