Job summary
Are you and experienced nurse will at least 2 years post registration experience? Do you have a passion for Palliative Care?
We are offering an exciting opportunity to join the established Palliative Care Support Team, we are looking to recruit an enthusiastic and motivated nurse with a passion for palliative care to lead and manage the service.
The Bradford and Airedale Palliative Care Support Teams look after patients in the last weeks of life. Our aim is to keep patients in their own homes if they wish to die at home with dignity and respect. The service works closely with the District Nurses, Specialist Palliative Care Nurses and Fast Track Team.
You will demonstrate excellent communication skills in supporting patients and their families and must be committed to providing high quality compassionate care.
You will be required to lead and support a team to provide high quality evidence-based care to patients living in their own home or care setting. You will manage a case load of patients and provide leadership to the team.
Main duties of the job
You will be responsible for the co-ordination of patients who are at the end stage of life in conjunction with the multi-disciplinary team.
Being personally accountable for professional and ethical actions and ensuring compliance with the NMC Code of Conduct
Maintaining confidentiality, whilst communicating patient information, in such a way that preserves dignity and privacy of the patient and family/carers
Working in a non-judgmental anti-discriminatory way, with regard to cultural and religious beliefs of individuals and groups
Ensuring that prior to any courses of action involving individuals/groups has their informed consent
Acting in ways that are consistent with the Trust's policies and values.
Acting as advocate for individuals and groups
Focusing resources to ensure equity of access for all individuals and group
The post holder will be required to work across a shift pattern between the hours of 8am to 9pm pattern in response to the needs of the service, over 7 days a week
