Job summary

An opportunity has arisen for a Band 6 registered Mental Health Nurse to join our dynamic, committed and supportive integrated, multi-disciplinary Community Mental Health Team in Bradford.

You will be working with adults with severe and enduring mental health needs to support their recovery journey working in partnership with service users and their families. This role will require assessment, planning, implementation, and evaluation of care, working directly as a care coordinator and also covering duty and clinics as required. You will also contribute to the supervision and development of junior colleagues. You will have recent experience of working in a mental health environment with 2 years post qualification experience and be able to demonstrate excellent communication and organisational skills and the ability to prioritise your workload.

Main duties of the job

Community Mental Health Nurse will provide community based care to clients with complex mental health needs who have been referred into the integrated Community Mental Health Team. He/She will work as an autonomous practitioner managing and prioritising individual caseload and will act as a Care Co-ordinator within the Care Programme Approach. The post holder will be responsible for the assessment of care needs and the development, implementation and evaluation of programmes of care for a defined caseload of people with complex mental health and/or social care needs.

The post holder will provide high standards of skilled mental health nursing care to individuals and their families within a defined geographical area. The role will include active participation in the service planning and development of the nursing and multi-disciplinary teams.

The post holder will provide clinical supervision and support to junior members of the team and actively promoting the values and policies of the service and the Trust. The Community Mental Health Nurse will be an active member of the multi disciplinary team and will work in close co-operation with secondary and primary care based professionals, other community agencies such as Housing, Probation, Police, Children Schools & Families and other Voluntary and Third Sector Agencies.
