Job summary

The Intensive Home Treatment Team at Airedale Centre for Mental Health are looking for a qualified Community Mental Health Nurse (Band 6) to join their busy team.

You will be responsible for the assessment, planning, implementation and evaluation of care to adult service users of all ages. This role includes, working with older adult mental health conditions in service users' home environment. You will also contribute to the supervision and development of junior staff and regularly be Shift Co-Ordinator for the team. This includes, taking and making referrals to and from other health professionals and acute care services.

You will have at least 2 years' experience as a registered mental health nurse. Ideally, working in an acute mental health setting. If interviewed, you should be able to demonstrate very good knowledge of relevant mental health practices specific to intensive home treatment team and all age adult mental health conditions. You should also be able to discuss guidance from NHS England / Department of Health forward planning for all age adult mental health service users.

Main duties of the job

  1. Job Purpose:
  • To provide care and interventions, risk and safety management and safeguarding of all age adult patients involved with the Intensive Home Treatment Team (IHTT).
  • To engage in regular supervision with the Team Manager/Advanced Nurse Practitioner. Discuss, reflect and find solutions to challenges experienced in the role.
  • To deliver a high standard and quality service which enables patients to be supported throughout their care pathway whilst promoting independence and protecting choice, dignity, privacy and safety.
  • To work in support of other allied health care professionals and services in the assessment, planning, and evaluation of care and treatment.
  • To communicate effectively with other services of the Care Trust, other health care professionals and with the patient family, carers/ visitors to ensure holistic care is delivered.
  • To supervise and delegate work to Band 5 Nurses, Senior Healthcare Support Workers and Healthcare Support Workers.
  • To occasionally be Shift Co-ordinator in the team. Including, triage of referrals, inpatient services link worker and covering Bed Manager during normal office hours.
