3. Main duties: Deliver the Healthy Child Programme 4-5-6 model as delegated by the HealthVisitor or School Nurse; to include Health Needs Assessment, development,delivery and evaluation of care plans and the provision of health advice andsupport to children, young people and their families as required. Plan and deliver evidence based skilled nursing care to children on theSchool Nursing and Health Visiting caseloads in accordance with Trustpolicies, guidance and procedures. Undertake Family Health Needs Assessment as directed by the Health Visitoror School Nurse ensuring prompt feedback of all client contact to theallocated Health Visitor/School Nurse. Take appropriate action in cases of suspected or actual child abuse and workclosely with Health Visitors/School Nurses and the Trust Safeguarding Teamto ensure that children are protected from harm. To be familiar with theTrusts Safeguarding Procedures and attend safeguarding updates inaccordance with the roles training matrix. Attend Case Conferences/Child in Need /Core Groups/Team around theFamily, and Integrated Care allocation meetings, as directed by the namedHealth Visitor/ School Nurse. Adhere to the NMC Code (October 2018) and Bradford District Care NHSTrust Policies & Procedures. Take part in duty responsibilities; dealing with tasks and phone calls fromother professionals and service users as required. Following appropriate training raise awareness of the Immunisationprogramme by informing parents and carers in accordance with the localpolicies and procedures, Undertake Maternal Mood assessments as delegated by the Health Visitorand support the developmental of positive parent-infant relationships andattachment. Assess and support Children, Young Peoples, and Families mental health,working in partnership with key stakeholders and specialist services toimprove outcomes and well-being. Work within the 4-5-6 model to deliver all levels of service (community,universal, universal partnership, and universal partnership plus) to delivereffective, needs led services to children, young people, and their families. Work in partnership with key stakeholders, communities, and families toimprove outcomes in accordance with District wide plans and servicespecifications, delivering on Key Performance Indicators as required Deliver services including training, in a variety of settings (e.g., home, school,community, GP) to a wide range of people (children, young people, parents,carers, other professionals, members of the public) in a variety of ways (e.g.,face to face, group, digital)Communication Promote and use good communication, employment practices and teamworking. Keep accurate and up to date electronic records, providing reports andreferrals as requested. Ensure that personal information for children, families, members of staff andall other individuals is always kept secure and confidential in accordance withTrust Policies and the Data Protection Act 1998.Page 3 of 11 Work collaboratively with statutory and non-statutory partners to meetidentified needs.Research & Development Activity Support the service in audit/research and to benchmark clinical practices toimprove quality. Participate in regular audits of caseload/workload to ensure the most effectiveuse of resources. Using evidence-based practice and on-going audit and evaluation to ensurethat all interventions achieve positive health outcomes. Undertake satisfaction surveys (FFT) of Service Users on a regular basis toensure the views and experience of children, young people and families arecaptured. Work within the Local/National Standards and Operating procedures andClinical Governance plans. Carry out projects for research as required. Enable, encourage, and support individuals and families to address issuesthat affect their health and social wellbeing. Represent individuals, families, groups, and communities interests when theyare not able to do so themselves. Ensure that children, young people, and their carers have access to theinformation they need in order to make informed choices about their health.4. Working as part of a Team To adhere to the Agile Working guidance. To adhere to the Lone Working Policy and ensure SystmOne diaries areaccurately maintained. Work in partnership with Health Visitors/School Nurses to formulate packagesof care underpinned by the Healthy Child Programme and provideintervention either in the home, school, childrens centre, or communityclinics.Page 4 of 11 Work closely with team members to implement and prioritise plannedprogrammes of health promotion, public health initiatives, education, andadvice, using the Make Every Contact Count approach. Deliver informationon all aspects of prevention of ill health to children, families, and groups. Participate in policy development to improve the service, and updateguidelines and standard operating procedures when required.5. Managing Self Utilise reflective practice and participate in Management, Clinical andSafeguarding Supervision. Work to the 6 Cs of Nursing and always demonstrate professionalaccountability and maintain a professional portfolio for re-validation. Participate in an annual Appraisal process to identify learning and trainingneeds in discussion with the Health Visitor/School Nurse. Undertake a period of preceptorship with named Health Visitor/School NurseAttend mandatory training and continuing professional developmentprogrammes in line with the NMC requirements and take responsibility formaintaining up to date knowledge and self-development. Comply with all Trust policies, procedures, and protocols. Pay regard to materials and equipment. Carry out duties with due regard to the Trusts Equal Opportunity Policy. Seek advice and support from your line manager whenever necessary.6. Staff Supervision and Support Provide support to your direct reports and team members in relation to theirpersonal health and wellbeing. Support the Team in the induction and orientation of new and junior staff andactively participate in their development. Support the Health Visitors or School Nurses in the induction, orientation, andtraining of pre-registration students. Under the supervision of the Health Visitor or School Nurse, act as amentor/facilitator for students whilst they are on placement with the team. Support and facilitate training of the wider skill mix team including trainingprofessionals and key stakeholders e.g., schools in the use of the Epi-pen. Contribute to, or undertake, staff appraisals as directed by the HealthVisitor/School Nurse Participate in events or workshops that aim to develop the workforce and/or theservice.7. Financial ResponsibilityThe 0-19 Community Staff Nurse post does not carry any financial responsibility inrelation to budget control.8. SafeguardingAll staff members have a duty to report any concerns they have about the safety orwell-being of adult service users, members of their families, including children.Employees should be aware of their roles & responsibilities to both prevent andrespond appropriately to abuse. They should undertake the safeguarding trainingrequired for their particular role.
This is a 0-5 service that is evolving and currently undergoing some exciting service changes and progression. There is a focus on integrated working with local authority partners and a whole team approach to providing early support to children and young people living within the Bradford and Airedale district. We are pro-active public health practitioners who work closely with children and families ranging from 0-5years, to support them in making healthy lifestyle choices and accessing other specialist services to meet their child's health needs. We are committed to the delivery of the Healthy Child Programme and ensuring our service delivery is of high quality and research based.
We are also involved in several new projects to support the public health agenda both with our local authority partnerships and with Public Health England. This is an exciting time to be joining the service as we are piloting the MECSH (Maternal Early Childhood Sustained Home Visiting) programme in the health visiting service in one locality of Bradford, the Better Start area covering Bradford East. This is a demanding specialist role requiring high levels of professional skills and practice which will all be enhanced by ongoing training regarding the delivery of MECSH and supported by MECSH Supervision. The MECSH (Maternal Early Childhood Sustained home visiting service add link) is a licensed preventative / early intervention programme for identified families. It has been developed in Australia and has been rolled out in some sites in the UK. Bradford is the first site in Northern England to trial this programme.
