Job summary

A rare and exciting opportunity for a band 6 community nurse has arisen within our Craven Children's learning disabilities Team. As a team we continually strive to develop our quality health care services and we are seeking an enthusiastic and highly motivated nurse to join our dynamic service.

As an individual we will expect you to possess excellent communication skills, enjoy working as part of a team, working closely with colleagues from a range of disciplines, whilst being able to work independently, carry out comprehensive assessments and plan, implement and evaluate delivery of nursing interventions as part of the holistic approach. You will also be able to demonstrate that you are able to apply your nursing skills to a wide range of situations.

Main duties of the job

1. At all times work in accordance with Bradford District Care NHS Foundation Trust policies and procedures2. Work in partnership with the Children's Learning Disability team and theSchool Nursing Special Needs team3. To provide a safe, responsive, efficient, timely, high quality communitynursing service to children and young people with learning disabilities, their families and carers4. To undertake a range of comprehensive assessments to children and young people with highly complex and challenging needs. This assessment work requires a highly specialist knowledge base on which to practice.5. To develop a range of interventions and specialised care plans. To develop a range of person centred, specialist interventions to meet the needs of children and young people with a learning disability.6. To co-ordinate, monitor and evaluate highly complex interventions andpackages of care within a defined caseload7. To facilitate clients and carers' understanding of highly complex issues inrelation to their presenting needs. The clients will have significantcommunication impairment which will affect the way they express andunderstand information in relation to their condition8. To promote, identify and facilitate the use of reasonable adjustments toensure health outcomes are optimised, this will involve collaborative working in primary and secondary health services9. To embrace public health role, promoting health and wellbeing and where possible, prevent ill health
