Dementia Outreach Practitioner - B6 Allied Health ProfessionalService: Older People's Mental HealthFull time: 37.5 hours

We are looking for a passionate and highly motivated specialist occupational therapist or physiotherapist to join our Bradford District Care NHS Foundation Trust Older Peoples Mental Health Service (OPMH) in a new Outreach Team which has been established to support residents in care homes (and those living in the community if and when needed) who require enhanced support and/or present in crisis out of the usual hours of our existing Older People's Mental Health (OPMH) service.
You will be joining a dynamic team which is structured as a B7 Gate Keeper RMN, B6 RMN, this post, a B4 therapy assistant practitioner and 2 x B3 HSCWs. The Outreach Team, hosted within the BRDCFT Dementia Assessment Uni, will work closely with care home providers across the B&C district who routine provide care for people living with dementia (or suspected dementia).
Main duties of the job
You will be working with the care home staff, our health and care professionals, and the individual and their family with the goal of supporting the individual to live well for longer in their usual residence with quality of life; avoiding the need for unnecessary transitions of care (e.g. to A&E/acute hospital admission) and/or heightened levels of formal care and support.
The Outreach Team will receive referrals from the Care Home Liaison team, other older people mental health professionals and potentially care homes directly - for individuals requiring additional support to prevent escalation of distress behaviours and where there is a need to support care home staff to continue to provide personalised care and support aligned to evidence based models of care, for example CLEAR Framework and the NHSE/I Dementia Well Pathway.
We seek an enthusiastic AHPs to join this new and evolving Outreach Team in recognition of the unique contribution of these professions in delivering holistic person-centred assessment and care and support for people living with dementia. AHPs have a unique role in supporting individuals with dementia to participate meaningfully in daily life, to be empowered to self-care and to use strategies (with the support of others around them) to avoid crisis and escalating distress behaviours through holistic assessment, goal - and/or occupational- focused rehabilitation and ongoing support.

As the B6 OT within the Outreach Team you will work as part of the MDT without our Dementia Assessment Unit. You will have a key role in developing the Outreach Team and in supporting continuous improvement to care quality through the development of AHP contribution within an outreach model of care wrapped around our existing care home liaison and support pathway. The role will involve working with partner organisations in the acute, voluntary and community sectors to achieve this aim and to deliver high quality person centred for older people living dementia and their families to improve their quality of life.
We are looking for an AHP who is passionate about the care of people with suspected and/or diagnosed dementia. You will have your own clinical caseload and a lead role in ensuring the development of others including those within our OPMH service and with care sector colleagues, so they too are more equipped with the necessary knowledge and skills needed to care for those living with dementia in their care, including those with co-existing physical and mental health conditions.
You will work with the OPMH AHP Lead and the OPMH clinical specialist physiotherapist/OT to effectively evaluate the role of OTs and AHPs, participate in service evaluation and show commitment to improving patient and family career experience.
