Job summary

BDCFT is looking for enthusiastic and innovative Band 6 Mental Health professional to be an active part of a team of trained and competent professionals in Acute Mental Health, who can deliver a timely and compassionate assessment which effectively sign postsservice users with mental health issues who are in the Criminal Justice System.

This role will be to provide a service in partnership with the Liaison & Diversion team, with key aims of jointly and compassionately signposting service users into appropriate care and support who are located within the local prison services. The successful applicant will be working alongside Liaison & Diversion as part of the Reconnect Programme for those identified within a prison setting who require support and increase access and uptake of healthcare or relevant support services for individuals who have health vulnerabilities on release from Prison, either on remand or on completion of their sentence

The successful applicants will need to be an excellent motivated practitioner, able to work within another team, respecting their views and methods, while simultaneously promoting Mental Health and collaborative working. In addition, close relationships and partnership working will be essential with other key stake holders such as the Voluntary Care Sector, Local Authority, Blue light services and above all the service users and their families.

Main duties of the job

  1. Job Purpose:

The aim of the service is to divert vulnerable people out of the criminal justice system and into services that can support them longer term.

West Yorkshire Liaison and Diversion (L&D) is an NHS England commissioned service delivered by Wakefield Council. This post is a partnership post with BDCFT.

The Reconnect Mental Health practitioner will work as a senior member of a multi-disciplinary team providing intensive mental health and social care assessments, to identify care pathways for individuals released from Prison. The Reconnect Mental Health practitioner will work in HMP Leeds, HMP New Hall and HMP Wealstun, undertaking intensive mental health and social care assessments, to identify those with health vulnerabilities and in need of support services on release and ensuring detailed care plans are in place.

The Reconnect Mental Health Practitioner will establish and maintain effective working relationships with senior delivery partners within the Prison, Local Authority, Probation, NHS, Police, Adult Social Care, Mental Health Services, Substance Misuse providers and Accommodation providers to coordinate care plans which will support individuals to engage with local services.
