Job summary

Barnardo's are leading the development and embedding of the West Yorkshire Children & Young People Keyworker Programme as a demonstrator site.

The Children & Young People Keyworker Demonstrator Site has been developed as a response to the NHS England & NHS Improvement Long Term Plan (LTP) commitment that by 2023/24, eligible children, young people and inpatients with a learning disability, autism - or both - with the most complex needs will have a designated Key Worker

The Keyworker pilot in West Yorkshire aims to provide dedicated key workers for children and young people with a learning disability and/or autism diagnosis who are already in hospital or in crisis and at risk of admission. The pilot will work with children and young people aged 0 - 25 with a focus on 14 - 25 years.

The keyworker pilot challenges systems to work collectively and across organisational boundaries to ensure support wraps around children, young people and their families when they most need it.

Bradford District Care NHS Foundation Trust will be hosting a Lead Role to draw together services and key stakeholders to ensure that the values and principles of the pilot are fully embedded and transforms the experience of children, young people and their families,

Main duties of the job

The post holder will work closely alongside the Barnardo's Services Manager to support the delivery of the keyworker ambitions and programme of work.

The postholder will work strategically across the local system, crossing organisational boundaries and unblocking traditional barriers to access, ensuring that keyworkers and the keyworker pilot is embedded within the pathways across health, education and Local Authority facilitating a keyworker for all children, young people and families that need it.

The post holder's objective will be to work in partnership with Barnardo's and across the system to ensure that the Children & Young People Keyworker demonstrator site delivers the outcomes as defined within the NHSE pilot requirements and is in line with the resourcing envelope as agreed within contract.

Acknowledging the complexity of navigating and bringing about significant change and transformation within statutory organisations, this role will act as an enabler of the pilot, facilitating cross organisational and system learning and change, creating a whole 'System' that will simplify and remove traditional barriers to accessing the right support at the right time for all families.

To provide regular and detailed reports to the related governance and accountability groups to include attendance and representation at regional and national Children & Young People Keyworker Demonstrator Site/Community of Practice meetings hosted by NHSE.

To arrange and facilitate project group meetings as required

To work in partnership with Barnardos to develop an evaluation plan support sustainability post pilot

Undertake a project risk assessment, ensuring risks are identified, managed, recorded and regularly reviewed.
