Job responsibilities

Main Duties and Responsibilities

Provide and monitor care in the community setting:

Provide compassionate, safe and effective care and support to people in a range of care settings.

Monitor the condition and health needs of people within their care on a continual basis in partnership with people, families, and carers

Contribute to ongoing assessment recognising when it is necessary to refer to others for reassessment

Communicate effectively with colleagues, providing clear verbal, digital or written information and instructions when sharing information, delegating or handing over responsibility for care

Recognise and report any situations, behaviours or errors that could result in poor care outcomes

Undertake lower limb re-assessments and wound management using the appropriate documentation following the full training and assessment of own competence levels by a regulated professional.

Record and analyse the following clinical tasks using existing clinical knowledge and experience; Measure and record blood pressure, temperature, height & weight body mass index, Purpose-T assessments, nutritional assessment using Must tool.

Undertake lower limb re-assessments using the appropriate documentation following the full training and assessment of own competence levels by a regulated professional.

2. Planning:

Working alongside senior health professionals in planning the clinics and visits, prioritising according to agreed protocols and seeking advice when required

Participating in co coordinating patient visits and planning reviews accordingly

Communication

To liaise as appropriate with members of the Adult community teams.

Develop good working relationships and liaise as appropriate with parents/ carers, and other health, social or voluntary agencies.

To communicate effectively with clients in complex situations. Communication skills of engagement, negotiation, motivation and explanation will be required to enable the management of potential barriers to communication.

To be actively involved in the communication of complex information regarding clients needs and any other useful information to the Adult Nursing Team, and/or other appropriate medical services (such as GP) and members of the wider Adult Services team.

Budgetary Responsibility

Take responsibility to minimise incurred expenditure e.g. Mileage, telephone calls use of equipment etc

Ordering and receipt of stock

People Management

Actively participate in annual performance reviews providing evidence of attaining the relevant knowledge and skills required for the job.

To attend mandatory and relevant training courses and to maintain up to date clinical knowledge and skills in order to perform in the role.

To participate in staff and service development through attendance at service meetings and in service training.

Clinical and Practice Governance

Observe and maintain strict confidentiality with regards to any information in line with the requirements of the Data Protection Act.

Any data that is taken/shared as part of a phone call or transported, faxed or transferred electronically must be undertaken with regard to the Trust Information Governance and Information Security policies.

The post holder must adhere to the Trust risk assessment and risk management processes.

To contribute in programmes and education support for staff and students.

To actively participate in programmes of supervision to maintain own development and reflective practice.

To actively engage in developing own practice through annual clinical observations.

The post holder must adhere to infection control policies and procedures.

The post holder must participate in clinical and safeguarding audits as required.

The post holder is required to participate in relevant emergency preparedness process for their team.

Qualifications and Training

Essential

• Foundation Level Degree: Nursing Associate Qualification
• NMC Registered Nursing Associate
• Educated to a minimum level 5 Foundation degree level specific to nursing associate practice
• GCSE Grade A-C English and Maths or Level 2 in Maths and English
• Completion of End Point Assessment for applicants who have undertaken an apprenticeship route

Desirable

• Care Certificate or willingness to train
• Related health and social care qualification (Level 3)

Experience

Essential

• Experience of working in a team as a registered nursing associate
• Experience of patient-centred care
• Experience of health coaching and promoting independent living

Desirable

• Previous experience of working within the community setting
• Evidence of involvement in support / development of less experienced staff
• Experience in community or hospital based healthcare.

Knowledge and Skills

Essential

• Ability to communicate with members of the public and health and social care providers
• Ability to deal with a non-routine and unpredictable workload and individual patient contact
• Ability to manage own workload
• Ability to take part in reflective practice and clinical supervision activities
• Understanding of basic physiology, e.g. normal vital signs, nutritional requirements, referral processes etc.
• Ability to work on own initiative
• Standard keyboard skills and ability to communicate through IT using packages such as Word, Outlook, Excel and PowerPoint.
• Report writing skills (referrals, Datix).
• Ability to work flexibly as part of a team.
• Time Management and prioritisation Skills and Willingness to learn appropriate clinical
• Understanding of evidence based practice

Desirable

• Competent with Systmone Willingness to learn appropriate clinical competencies

• Demonstrates understanding of safeguarding issues

• Has a good understanding of the Safeguarding agenda
• Can demonstrate an ability to contribute towards a safe environment
• Is up-to-date with legislation and current events
• Can demonstrate how s/he has promoted 'best practice'
• Shows a personal commitment to safeguarding children
