Job summary

Join our school nursing team as we reinvigorate our service , offering a visible school nursing team , back into schools to ensure we are offering early intervention , health promotion and needs lead interventions.

Main duties of the job

The post-holder will work within a clinical governance framework which is embedded in day to day practice. CCS, Bedfordshire / Luton Community Health Services is accountable for continuously improving the quality of its services, and safeguarding standards of care.

Take every opportunity to promote and participate in health promotion initiatives.

Act as a signpost for families to other relevant services thereby empowering them as users.

Maintain a safe environment for patients, carers and staff where possible, ensure in the implementation of evidence based practice.

Actively develop a more specialised service or activity which is based on clear evidence of need, in partnership with commissioners.

Provide the core school nursing programme to the family and community.

Work in partnership with clients and families to agreed contracts in order to promote and maximize physical, emotional, psychological and social well-being.

Undertake and provide a holistic assessment of client's health needs and to produce, implement and review the care plan.

Review and evaluate outcomes for children and families.

Refer clients to other agencies as appropriate e.g. onward referral of identified vulnerable children and adults.

Develop with other professionals an overall specialist protection plan where indicated e.g. Child Protection Plan.

Knowledge and Skills

Essential

  • Relevant expert knowledge
  • Experience of working in partnership with other relevant services e.g. CAMHS, specialist perinatal mental health, IAPT and 3rd sector organisations.
  • Health needs assessment and identification experience in relation to older children
  • Experience of working with children and families in Safeguarding arena
  • Knowledge of Public health agenda both locally and Nationally
  • Ability to Safeguard and promote the welfare of children and young people/vulnerable adults

Desirable

  • evidence of undertaking audit

Qualifications and Training
