The Specialist Children's Community Nurse- Complex Clinical Needs will work within the Children's Community Nursing Team in the community settings across Bedfordshire and Luton.

You will lead, develop and implement pathways for the nursing care of babies, children and young people with complex clinical needs across the Children's Community and Specialist Nursing service.

Supporting teams embed the relevant pathways into their clinical practice, and to provide education and training using your expertise.

It will essential to develop positive working relationships within an integrated community network to influence patient and professional experiences to ensure best outcomes.

Main duties of the job

Be an expert resource for Children's Complex Clinical/ skills and knowledge, providing education and training to the CCNSN Team and the wider multi-disciplinary team.

You may be required to participate in the on-call rota for the service.

Be the lead Clinician within the CCNSN Team for the specialised area of children's complex clinical care; including networking with specialist nurses to develop practice and sharing knowledge and skills with colleagues.

Work in conjunction with the named professionals for children/young people with complex clinical, acting as a resource to improve healthcare outcomes

Lead, develop and establish integrated clinical care pathways across CCNSN services to ensure a consistent approach to case management/care coordination.

Work with the Children's Services Team Managers ensuring that the resources and skills of the team are used effectively and appropriately to meet the needs of the children/young people and their families, helping to identify any gaps in service provision.

Liaise and work collaboratively with professionals in primary, secondary and tertiary healthcare settings to ensure timely discharge planning.

Co-ordinate/facilitate the implementation of complex symptom control regimes; plan and lead specialist complex community nursing interventions utilising a range of clinical skills.

Support the implementation of transition pathways from Children to Adult Services for children with complex needs, ensuring a seamless pathway

Maintaining own professional development and requirement to take part in appraisal and KSF process.

Observe and maintain strict confidentiality with regards to any patient/family/staff/records and information in line with the requirements of the Data Protection Act.

Any data that is taken/shared as part of a phone call or transported, faxed, or transferred electronically must be undertaken with regard to the Trust Information Governance and Information Security policies.

The post holder must adhere to the Trust risk assessment and risk management processes.

The post holder must adhere to infection control policies and procedures.

It is a condition of your employment that you are currently registered with your professional body, and it is your responsibility to maintain your professional registration.

Undertake mandatory training and any other training relevant to the role as required by Cambridgeshire Community Services NHS Trust.
