Job summary
A new exciting opportunity has arisen to join a well established community epilepsy nursing team. You will be well supported by the Epilepsy Nurse Specialist and the Assistant Practitioner within the team to deliver community care for patients with epilepsy.
To assess, evaluate and deliver programmes of nursing care for the epilepsy specialist caseload of children with nursing needs in the community of Luton and South Bedfordshire.
The purpose of this role is to facilitate early discharge and to prevent admission/readmission to hospital and to develop selfcare for caseload of children by providing Support nearer to home.

Main duties of the job
To support the lead nurse in service provision in support of children, young people and families who have increased needs with regards to their epilepsy management.
You will be supported by the Epilepsy Nurse Specialist to assess, plan, implement and evaluate individual programmes of care for a caseload of patients requiring treatment for Epilepsy.
To support the transition of young people from paediatric to adult care, this may include attendance at transition clinics, liaising with the adult neurology team and in-reach to adult wards as appropriate, for young people in the process of transition.
Liaise with multidisciplinary teams within hospital (paediatric and adult), community, primary, and social care, including schools and nurseries. Patient contact- work in partnership with children, families and carers to provide the optimum individualised nursing care in the most appropriate settings eg. Home, school, community clinic.
Policy development- to participate proactively with the Epilepsy Nurse Specialist to develop and improve the service. To update guidelines and Standard Operating Procedures as required. Maintain reporting of relevant data to national surveillance projects and local audits.
