Job summary

You will be responsible for day to day case management of non-registered staff ensuring delivery of individualised patient care, through holistic assessment, care planning and evaluation of care to achieve positive health outcomes for our unique and complex children.
You will ensure that all young people receive health care promoting a valued lifestyle, supporting independence where possible so they can attend nursery/school /community settings encouraging them to reach their full potential.

Main duties of the job
To assess, implement, deliver and evaluate programmes of nursing care to children and young people adopting a flexible holistic approach to meeting individual needs. Supporting children / young people and their families in meeting identified health needs, either in the family home, or other community settings as appropriate, in collaboration with other multi agency providers across BLMK.
To plan and deliver evidence based skilled nursing care to children on the CCCT caseload. Be able to write comprehensive care plans.
Work in partnership with children, families and carers to provide the optimum individualised care in the most appropriate settings.
Co-ordinate service provision for Children's Continuing Care Team across both Bedfordshire and Luton.
You will help to ensure that policy and practice is developed and delivered in line with Trust and national best practice standards.
You will deliver planned teaching and assessment of competence for families, nurses, and non-registered professionals. You will provide the mentorship and supervision of student nurses, Preceptorship of Staff Members, and mentor Student Nurses,
You will line manage qualified/non-registered staff, Act as a role model to junior staff members including undertaking the assessment of clinical competencies, this could occur day or night across our 7 day service.
