Job summary
An exciting opportunity has arisen for an experienced, enthusiastic and compassionate nurse to join our growing Macmillan community cancer CNS team within Luton.
In order to deliver the best care for patients with a cancer diagnosis closer to their homes we are looking for a nurse who is compassionate and driven to make a difference to patients' lives who are living with and beyond a cancer.
You will work within the community setting in Luton, which is culturally rich and diverse, providing care to people in their own homes, clinics and wider community settings
You will be part of a specialist nursing team supporting other community nurses in various settings and GP practices to ensure the appropriate support is available to patients with a cancer diagnosis. You will work closely with colleagues in The Luton and Dunstable NHS University Trust, the community palliative care team and other local health and social care services.
You will have clinical experience and an understanding of the needs of patients living with a cancer diagnosis and experience of delivering education. You will have management and leadership skills and be part of a team that is keen to develop and move forward.
You will be expected to contribute to the wider cancer agenda, ensuring a flexible responsive service that meets the local needs within Luton.

Main duties of the job
The post holder will work as part of a multidisciplinary team providing specialist cancer nursing advice and care. They will be expected to work with a wide range of health and social care professionals within the acute care and community setting.
Making appropriate clinical decisions independently and seeking senior guidance where necessary in reviewing, monitoring and managing cancer patients.
To proactively plan, co-ordinate, deliver and evaluate the provision of a high quality specialist cancer nursing service within the community and across the primary and secondary care interface.
To work with acute service provider, plan and consider the treatments that could be delivered in the primary care / community setting in the future and prevent emergency presentations
To formulate, monitor and evaluate evidence based standards and policies relating to cancer care.
Provide support and be proactive in educational training and mentorship to the generalist workforce in line with trust requirements.
To ensure seamless transfer, when appropriate to palliative care services.
