Job summary

At CCS we are always looking for highly motivated Specialist Community Public Health Nurses to join our Bank staff. We offer flexibility to work the times and days that suit you - outside of your normal working hours with us or elsewhere, or as your only employment. Joining our locality 0-19 team within the Healthy Child Programme in Norfolk in a Bank role allows you to have control of your working life while also helping children and families in our local communities

The successful candidates will be responsible for assessing need, delegating work within the team, implementing planned programmes of care and evaluating care delivered.

This role will provide a child centred, inclusive, holistic and accessible Public Health Programme to children, their families and communities, establishing links with Early Childhood Family Services bases, Early Year's Settings, Early Help, other health providers, communities and agencies (Education, School Services, Health and Voluntary organisations).

Job description

Job responsibilities

To work as part of a multidisciplinary team to identify the health needs of children and young people, their families and the community and contribute to developing a strategy to address individual identified needs as well as those needs within the community.

To be confident to use telephone triage skills and knowledge to assess, plan, implement and evaluate packages of care.

To be competent as a non-medical prescriber.

To provide home visits and child health drop-ins to assess the needs of children and their families, during a rotation period into locality 0-19 teams.

To support children and young people and their families with emotional and behavioural development.

To advise their client group on the promotion of health and the prevention of illness.

To link practice with local and national targets and deliver public health programmes based on current priorities and targets.

To plan evidence based health promotion programmes, order resources, compile teaching plans with aims, objectives and outcomes, and evaluate the sessions.

To act as a health link for other agencies, e.g. Early Childhood and Families Service, GPs, voluntary services, public and private agencies and work closely with partner agencies to promote the health and wellbeing of children, young people and families.

To work with the locality teams to support children, young people and families who are considered to be vulnerable or at risk. To adhere to current Safeguarding Procedures, and participate in child protection case conferences and case review meetings as required.

To contribute to the development of local policies and procedures relevant to their area of work.

To take responsibility for their own clinical practice including clinical supervision and contemporaneous record keeping, adhering to the NMC code of conduct.

To participate in meetings and attend relevant training courses to demonstrate ongoing professional development.

To engage in Service Design developments.

To support the digital delivery of the Healthy Child Programme and promote this with Service Users and professional agencies.

Within this role you will need to travel as an independent practitioner on a daily basis, across Norfolk which involves both rural and urban travel. The nature of our work does result in travel arrangements changing at short notice. Due to the large geography our service covers and the limitations of sufficient public transport to undertake this role you must hold a full and current driving licence and have access to your own transport to meet the travel requirements of the role. This is subject to the provisions of the Equality Act 2010, and therefore if you require a reasonable adjustment due to a disability then this will be considered.

To communicate complex and sensitive information concerning the child or young persons condition to the childs parents/carers and young person. Able to communicate effectively using appropriate methods and recognising barriers to communication.
