We are delighted to be able to offer the opportunity of a Service manager for the Integrated District Nursing Team.
This post provides an exciting opportunity for an enthusiastic, strategic and dynamic individual to join our community team.
Working with partner organisations to deliver truly integrated community based services in line with the NHS long term plan priorities to improve patient outcomes through a population health management approach. This approach aligns to the 'place' commissioning of enhanced models of care to mitigate the challenges of mild to moderate frailty, management of dementia and supporting people to be as well as possible as long as possible through supporting proactive management of long term conditions. .
The post holder will play an integral role in enhancing multidisciplinary working and workforce development through primary care networks, acting as a focal point of contact and PCN relationship manager.
This role brings together those aspects of care that ensure people are well supported to stay at home under the care of their own GP and truly put the patient at the centre of all we do.
Practice Nurses, therapists and our partners locally will be part of the virtual wards this role will be party to supporting. You will be part of the system leaders looking at innovative ways of working to share best practice and ensure that the patient, their carers and family are cared for as close to home as possible.
Main duties of the job
- To ensure that the service delivers against the service specifications effectively and safely meeting all quality
- Develop partnership working with other local providers to build proportionate responses for patients locally to enable them to remain at home or a community
- To engage and where appropriate lead on key aspects of clinical service development in partnership with the Trusts service redesign team
- To ensure that the clinical standards for the service are established in line with local and national best practice.
- To monitor performance of the clinical leads and ensure that patient safety is managed effectively and risks are
- Manage and respond to complaints and adverse
- Ensure that the service is available 24/7 365 and responds effectively to peaks in
- To develop integrated working across the adult and children's team where people are transitioning to adult care
- Ensure that the services have in place clear procedures, processes and are compliant with Trust policies, CQC standards and commissioning quality
- Actively participate as required in local and regional resilience meetings and delivery of the services business continuity
- Work with the other service leads to develop and support patient engagement and involvement programmes to improve outcomes for local people and reduce admissions and attendances at A&E.
