Job summary
We are expanding our Rapid Response Community Nursing Team in order to meet the new Urgent Crisis Response (UCR) and Virtual Ward requirements. This brings an exciting opportunity for experienced Registered Nurses to join the team.
Virtual wards are a national initiative that will support patients who would otherwise be in a hospital setting to remain in their own home environment. This will be supported with the use of digital technology, monitoring, supporting the setting up of equipment and providing patient care and the treatment they need. It is expected that patients will move between UCR and virtual ward pathways.
The Rapid Response Team is an established and successful team that provide high quality nursing care to the people of Luton to enable them to maximize their health status, minimize the effects of ill health and disability, and to lead as independent a life as possible.
To assist in the assessment, planning and implementation of holistic care provision.
To work with the MDT to prevent avoidable hospital admissions .
Main duties of the job
We provide a comprehensive 24 hour urgent/unplanned Nursing Service and are currently looking for motivated proactive staff nurses to support the D.N teams in order to meet the health needs of patients in Luton as part of the Out of Hours (OOHs) Rapid Response Team. As an experienced nurse you will be adaptable to overcoming challenges and ensuring that the patients receive the high quality care they require and further refer onto other services that are identified to be appropriate. Supporting other colleagues and embracing service development is fundamental to the role.
- Our Community Teams work autonomously and are required to visit patients in their own home including residential homes for urgent / unplanned care.
- Healthcare needs you will provide include; unplanned visits for catheter and wound care and post hospital discharge visits ,
- Our team visits patients registered with a Luton GP. And also supports patients on the District Nursing caseload out of hours.
Experience of working in Luton would be an advantage, although full induction and training will be given in line with service need and therefore will be required to develop your own competencies relevant to the local populations diverse health needs. Previous community nursing experience and holding palliative care and IV therapy competencies is desirable.
