Dudley Integrated Health and Care NHS Trust (DIHC) is the first Integrated Care Provider in the country. It was launched in April 2020 following a period of development and integrates primary care with community services within the Dudley healthcare system. Our core aim is Dudley First, community where possible and hospital where necessary, and our purpose is to improve the health and wellbeing of our local population in Dudley
We have an amazing opportunity for an enthusiastic nurse to join our new team of clinicians, supporting our Enhanced Care Home Service, whilst we in turn support your personal development with training and education opportunities.
We are also hoping to appoint 2 successful candidates to this role 1 candidate will be full time hours and the second will be part time hours.
Main duties of the job
The duties and responsibilities are intended to be indicative but not exhaustive of the responsibilities of the post holder. As Dudley Integrated Health and Care NHS Trust develops, the requirements of the job may develop and change.
However, to provide an insight into this role, the successful applicant could be expected to be involved the below:
- To include ward rounds at nursing/care homes to deal with clinical queries some which may be urgent to avoid admission, medication reviews, referrals and end of life care.
- Ensure the highest standards of treatment and care for all patients (including complex patients, both acute and chronic). This will involve supporting diagnosis, implementation of care plans, to ensure physiological and psychological factors are taken into consideration and ongoing evaluation of any plans to ensure patient compliance.
- Working closely with the EHCH Team and our team of Pharmacists to prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice.
- Health promotion and motivation of patients to improve general health.
