Dudley Integrated Health and Care NHS Trust (DIHC) is the first Integrated Care Provider in the country. It was launched in April 2020 following a period of development and integrates primary care with community services within the Dudley healthcare system. Our core aim is Dudley First, community where possible and hospital where necessary, and our purpose is to improve the health and wellbeing of our local population in Dudley.
The post holder will deliver a high standard of patient care and assist with patients across all GP practices that we serve. Your role will be to help our patients to get the best out of the local services including local charities, third sector, council and GP surgeries/ other NHS services. This also includes facilitating the creation of local groups
Main duties of the job
An extensive list of the main duties for this post can be found on the attached job description however as a Social prescriber for the PCN you can except to undertake main duties will such as:
- Aiming to reduce social isolation
- Offer financial help to our patient group
- Offer support to carers and family guidance
- Signposting patients to the appropriate services
- Create social groups within the PCN
- Create teams of helpers and volunteers for the elderly i.e. shopping, bringing to GP appointments, travel to social groups
- Meet with and follow up patients to develop and review a relevant social prescribing care plan.
