Dudley Integrated Health and Care NHS Trust (DIHC) is the first Integrated Care Provider in the country. It was launched in April 2020 following a period of development and integrates primary care with community services within the Dudley healthcare system. Our core aim is Dudley First, community where possible and hospital where necessary, and our purpose is to improve the health and wellbeing of our local population in Dudley.
High Oak Surgery serves a practice population of approximately 4000 patients, mainly within the Pensnett and Brierley Hill areas of the Dudley borough, and is currently located within Brierley Hill Health and Social Care Centre, a modern purpose built building. Chapel Street Surgery serves a list of around 2500 patients in the multi-ethnic community of Lye.
Both sites are traditional surgeries: friendly practices with great staff morale and a pleasant working environment. The workload is busy but manageable with structured admin time, limited extra patients and defined on call duties. We have very low home visit rates and minimal care home residents.

We are an APMS practice serving about 4500 patients working from Brierley Hill in Dudley.
