Job summary

This is an exciting opportunity to join our community Palliative care Rapid Response Team

This team has been started and developed over the last 12 months, and is now flourishing, providing excellent specialist urgent complex palliative and end of life care to the population of Coventry in a timely 2 hour response

Coventry Palliative Care Team supports patients, families and carers with specialist palliative care needs, palliative support, and end of life care within the community setting. The post holder will be required to work as part of the integrated palliative care multidisciplinary team and will work with other health care professionals to provide specialist advice, support, and high-quality patient centred care to those affected by malignant and non-malignant disease at the end of life within a 2-hour rapid response. This will be provided 7 days a week 8am-8pm on a shift rota basis.

  • Recordable teaching qualification

Experience

  • Substantial post registration clinical experience
  • Demonstrable experience working at a senior level in an appropriate clinical area
  • Can demonstrate experience of managing palliative care patients
