Job summary

The post holder will be required to manage the role and functions within LD community team service, ensuring adequate skill mix and safe staffing levels are maintained. This will provide a service to people with LD who require assessment to determine the required interventions to maximise recovery and independence.

The post holder will have responsibility for the operational delivery of the service and liaise with other partners in the pathway to ensure seamless service for service users and their families.

The post holder will:
