Community Nurse Children's Intensive Support Team - Band 6
Base: Paybody Coventry; but will be required to travel across Coventry and Warwickshire
Full time hours -To support families at key times, some work may be required outside of usual office hours.
An exciting opportunity has arisen for an enthusiastic and motivated community Mental Health or Learning Disability Nurse to be part of our Children's and Young Adults Intensive Support Team for Coventry and Warwickshire. The Children's and Young Adults Intensive Support Team is a specialist, multi-disciplinary team currently consisting of Occupational Therapists, Clinical Psychologists, Nursing and Support Workers working across Coventry and Warwickshire.

As a Nurse you will work closely with families and the wider multi-agency teams such and social care and education to improve the quality of life for young people and their families.

Coventry and Warwickshire Partnership Trust provides a full range of expanding physical, mental health and learning disability services to children, young adults, adults and older adults over many sites across Coventry and Warwickshire.
Services are provided to a population of over one million people living in Coventry and Warwickshire, and also a wider geographical area in some of our specialist services. We see on average around 5,000 patients every day.
The Childrens and Young Adults Intensive Support Team works across Coventry and the whole of Warwickshire (north and south) to provide additional therapeutic support for the families whose young people are most at risk of an admission to a Tier 4 hospital.

As a team we work systemically to understand and support the significant impact and challenges experienced by children and young adults with autism and/or learning disabilities, and their families. In order to do this, we work with the network around the child (e.g. Social Care, Education) and work closely with commissioners. As a community nurse you will have experience of multi-disciplinary and multi-agency working as you will need to attend and chair regular MDTs and attend CETRs and EMDTs.
As a nurse you would visit young people in home / at school / in appropriate setting multiple times a week to engage them and carry out our assessments and interventions. The nurses currently in the team describe this as the best part of the role and no day is the same.
Good working knowledge of ASD is essential for this role and an awareness of how communication challenges may impact on mental health.

- Professional registered qualification relating to learning disability, autism and/or mental health
- Diploma / Degree holder
- Evidence of ongoing CPD
Knowledge and skills
Essential
- Knowledge and understanding of up-to-date legislation/guidance in respect of learning disability and/or autism and/or mental health
- Knowledge of Positive Behaviour Support and a willingness to undertake further training to develop skills
- Demonstrate understanding of responsibilities of a registered professional according to professional body codes.
- Knowledge of Valuing People and the Transforming Care Agenda and their application into practice.
- Highly effective communication and liaison skills.
- Clinically proficient and able to provide direct assistance to service users with complex needs.

- Registered professional qualification with a substantial post registration experience at Band 5 or above.
- Experience of working in multi-agency teams with multi-disciplinary colleagues.
