Job summary

Are you newly qualified or due to qualify this year? If so an exciting opportunity has arisen to recruita community care coordinator (at Band 5 or equivalent) to join our multi-disciplinary Early Intervention in Psychosis Team. This position is available to a Nurse, AHP or Social Worker.

As this is a specialist team it is essential that you are passionate about working with individuals who are experiencing psychosis.

The Early Intervention in Psychosis service is an evidence based specialist service which works in line with the access and waiting time standards to deliver holistic interventions.

The successful applicant will be required to care co-ordinate 15 (maximum) patients as well as contributing to specialist CAARMS assessments alongside either Band 6 Clinicians or our medical colleagues.

You will be expected to contribute to the development and promotion of the Early Intervention Team and support the delivery of NICE interventions and the NHS England access and waiting time standard. There will be an expectation that you will provide psycho-therapeutic interventions such as CBA, BFT, psycho-social education, carers groups, working with families and early warning signs in order to assist the client group, families and carers in the management of their illness and its symptoms. There is an expectation that you will undertake training that will support the above psycho-social therapeutic interventions.

The ability to travel during the working day over a large geographical area is essential as is the ability to work flexibly with the needs of the client group. . You will need to have excellent interpersonal and problem-solving skills and will be expected to demonstrate your ability to work within a team. You will be expected to supervise support staff and students. The post is subject to DBS clearance.

  • Relevant Professional Qualification ie. RMN, DipCOT
  • Current unrestricted registration with HCPC/NMC
  • Specialist knowledge across a range of procedures and theory
  • Professional knowledge acquired through degree/ diploma
  • Relevant training and experience specialist short courses
  • Evidence of continuing professional development

EXPERIENCE

Essential

  • Substantial experience in a Community Mental health care setting / Psychosis
  • Developed specialist knowledge gained within a similar environment over a considerable period of time
  • Significant experience of undertaking assessment, formulation, risk assessment and positive risk management
  • Experience of safeguarding and investigations
  • Proven experience of people management

KNOWLEDGE & SKILLS

  • Excellent written communication skills

  • Must have full driving licence and access to a motor vehicle for business use
