Memory Assessment & Community Dementia Services within Coventry and Warwickshire.

Band 5 Registered Nurse

37.5 hours per week, permanent role

We are seeking to recruit a Band 5 Registered Nurse in our Memory Assessment & Dementia Pathway or our Coventry and Warwickshire locality.

We are looking for a motivated and dynamic individual who has a commitment to the positive development of this service, whose aim is to support people to live well with dementia.

Have a commitment to provide a responsive, flexible and evidence based specialist service to those people who are undergoing an assessment of memory problems or those diagnosed with a dementia being sensitive to their complex needs and presentation.

Main duties of the job

Your role will include

Providing post diagnostic support and post diagnostic interventions for those people who have been assessed and diagnosed with a dementia by the service

For those who meet the criteria for prescription of cognitive enhancers your role will include supporting the patient during the titration process to the therapeutic dose once the medication has been initiated.

To conduct the annual review for patients prescribed cognitive enhancing medication

You may also be involved in being part of the treatment plan proving interventions to clients (and their carers) who have an existing diagnosis of dementia with complex presentations.

To offer cover to our other community Dementia Teams within the Trust within as required

Alongside this you will be part of a team that

Signposts the individual and/or their carer/family members to appropriate resources in the community

  • Educates both client and carer
  • Offers appropriate post diagnostic support groups
  • Provides 1:1 support
  • Promotes Positive Risk taking
  • Most importantly involves and empowers both the individual and/or their carer to make decisions about their care and treatment now and for the future
