Lead Primary Care Mental Health Practitioner (ARRS) - Band 7

Full time, permanent role

This is an exciting opportunity to build on the existing mental health practitioner offer across the primary care networks (PCNs) to support GPs across South Warwickshire

We welcome and encourage applications from all professional backgrounds: Social Workers, Occupational Therapists (OT), Registered Mental Nurses and Psychologists.

This role will provide patient contact, clinical support, leadership, and management to the mental health practitioners that will be supporting the 7 PCNs across South Warwickshire.

These are exciting roles for people who are proactive and passionate about innovation and providing a high-quality holistic approach to the care of people who have been identified as in need of mental health support in a primary care setting.

It is a fantastic opportunity for someone to develop their skills and be part of developing an innovative and transformative service. Candidates will be required to have excellent communication and relational skills and be able to work with a broad range of people across the PCNs and CWPT.

Main duties of the job

The key elements of this role are:

To improve the management of mental health conditions in Primary Care through working closely with General Practitioners and the practice team.

Provide an opportunity to strengthen joint working, relationships and communication across primary care and adult secondary care community mental health systems, making it easier for patients to obtain the help they need earlier.

Work as part of new evolving teams that will be able to support locality PCNs equitably.

You will contribute to delivering and co-developing the primary care mental health service model. This includes care pathways for the provision of integrated mental health care within the Primary Care Networks and Coventry and Warwickshire Partnership trust

You will work as part of the MDT to support and advise the Primary Care Team in triage, assessment and treatment of patients needing mental health support.

As a lead practitioner you will manage and supervise a team of Band 5 and 6 Primary Care Mental Health Practitioners and coordinate the day-to-day operational / clinical issues arising from within mental health service delivery.

As a clinical lead you will also work closely with the PCN service lead and lead psychologist (when they are in post).

The post holder will be a resource of clinical knowledge and competence in the service area.

Primary Care Networks (PCNs) are groups of GP practices which work together in a local geographical area. In Coventry and Warwickshire there are 21 PCNs with a variety of different sizes, patient populations and mental health priorities. From 2021, we having been working toward ensuring there are mental health practitioners working across the PCNs.

Main Tasks:

Be highly proficient in the assessment, identification, treatment and management of mental health.

Provide advice and support to service users, carers and partner agencies in regard to people with mental health problems.

Ensure that priorities are identified in each PCN and that processes are in place to ensure effective service delivery.

Liaise and communicate effectively and sensitively with all other professionals in the wider PCN and MDT and external agencies (e.g., Older Peoples teams, Forensic Services, Drug and Alcohol) providing specialist advice as necessary.

Develop close working relationships with other agencies involved in current cases, the requirement for confidentiality, to promote complementary working relationships and ensuring a quality service regarding client care.

Have a good understanding of the specific needs of the service user group including at times of crisis, including supporting staff to identify signs of mental health relapses and contingency plans.

Work collaboratively with service users, carers and families as appropriate, to make decisions relevant to the service users individual needs.

Have highly developed skills in conflict resolution and positive behaviour approaches.

Develop, implement and evaluate specialist programmes of care which support the health and well-being of individuals or groups with mental health and complex needs.

Write timely professional reports/ records and action plans in line with Nursing and Midwifery Council guidelines that comply with Trust policies.

Work to reduce health inequalities for service users by leading partnership working with partners in primary, secondary care and County Council in implementing the local health agenda.
