This post offers an opportunity to work with the community CHD and Heart Failure nursing service across an identified area in a non-patient facing clinical admin role supporting the clinical teams.

Main duties of the job

To work with the community CHD & Heart Failure nursing service across an identified service area in a none patient facing clinical role:

o To be responsible for the accepting and completing of electronic tasks with the help of the nursing staff as required.

o Allocate work appropriately using electronic systems.

o To identify and apply appropriate care plans, setting frequency and date.

o Carry out administrative and housekeeping tasks to support the service and patient care.

o Participate and provide training around the electronic patient management system and functioning of the team in their area.

o Provide and receive routine information requiring tact or persuasive skills; Provide and receive complex or sensitive information; Provide advice, instruction or training to the team Provide and receive complex, sensitive information; dealing with and understanding behaviours

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.

Job responsibilities

Electronic referrals and tasks, action appropriately whilst seeking advice if required.

Manages own workload with an understanding of what needs to be done, when and why in order to assist the function of the team. Has the ability to flex and prioritise if required due to an unexpected or urgent need.

Provide staff with advice re administrative procedures and help with access as required. This includes general admin duties around RA agent and IAA functions.

Coordinating clinical work for colleagues, within SystmOne. This will include a review of allocated visits to ensure they are as geographically close together as practical while maintaining continuity of care to our patients.

Communicate with patients, relatives and health care practitioners effectively who may be distressed and/or challenging at times.

Demonstrating effective time management skills and participates in the monitoring and reviewing of team performance.

Gather key audit data from electronic systems and produce reports to Clinical Lead/Matron as requested and within time scales.

Promote an environment where health promotion and informed patient choice is encouraged and developed to its maximum potential, within the available resources

To undertake relevant training to enable the provision of a high quality service.

To complete effective record keeping in accordance with Trust policy and legal requirements.

To commit to continuous personal development/lifelong learning by attending mandatory training, relevant training courses , use of eLearning and adhere to all trust policies and procedures.

To attend supervision/1:1 sessions, to undertake appraisal preparation and contribute to objective setting and personal development planning.

To participate in and contribute to all activities associated with the teams response to current trends and changes within the NHS
