Job summary
  We
are currently seeking a Registered Nurse to Join our Specialist Lymphoedema
service. The successful applicant will work countwide, visiting patients in
their own homes supporting Community
Nursing Services with the management of lymphoedema from an office base
in Peterlee. The team work between the hours of 08:30-17:00 Monday to Friday.
The role involves assessing patients and providing specialist advice,
signposting where appropriate. Successful applicants will be required to hold a
first level nursing registration with the NMC, additional training specific to
the role can be provided.
  Main duties of the job
  To assess patients needs and develop, implement and
evaluate programmes of care.
To organise the delivery of lymphedema care.
To be aware of, and communicate to team members, changes to
patient information and treatment plans, both verbally and in writing.
To adhere to NMC standards of record keeping.
To assist the Band 6/7 in setting, monitoring and
maintaining standards of care.
To participate in nursing audit and service improvement
activities 
To be aware of resource constraints and the importance of
working within budget.
To provide advice and act as role model to other members of
the team.
To be proactive in managing risk and maintaining patient
safety
To contribute to the day-to-day management of the
Directorate, co-operating with colleagues to provide an appropriate level of
nursing service. To participate in education and training of other staff.
To involve patients/carers in the planning and delivery of
care.
To participate in clinical supervision.
To assist in resolving conflict and defusing situations
where appropriate.
