Job summary
Community Stroke Rehabilitation are looking for an
experienced qualified therapist with significant experience in stroke
rehabilitation to join our team. This Clinical Lead that is open to an AHP with
fast stroke knowledge and authethitic leadership skills.
The post will be based at the Sedgefield Hospital site and
will compose of both a clinical and a leadership role within the team. The
successful candidate will work with and support Occupational Therapists,
Physiotherapists, Speech and Language Therapists and skilled associates to
deliver innovative, evidence-based stroke rehabilitation in patients own homes
and out-patient settings to optimise recovery.
The role is diverse and challenging and we are looking for
someone to join our friendly team and to meet the needs of patients and bring
ideas to the service regarding service delivery and development. This is an

To undertake clinical role as an expert practitioner and
active provider of specialist clinical care, maintaining clinical skills and
demonstrating an effective role model for the therapy staff and other
professionals with in the multidisciplinary team.
To provide high quality care to an independent caseload
referred with complex multi-pathologies.
The post holder is an autonomous practitioner, using highly skilled and
specialised diagnostic, assessment and treatment techniques within the
speciality. The client may present with
acute, palliative, complex and / or chronic presentation.
To assess, diagnose, implement and evaluate individual
programmes of care for patients, including organising short term additional
care support as required / escalating care when crisis management at home is
not possible.
To support the wider therapy service resource providing
in-patient and community rehabilitation and case management
To ensure access targets are met and will actively
contribute to identify and problem solve issues as they occur.
To coordinate and monitor the delivery of high quality care
that, meeting identified patient need, supports the aims and objectives of the
integrated multidisciplinary team, and remains within the boundaries of
available resource.
The post holder will deliver clinical education of staff
within the therapy services and wider MDT
The Clinical Lead needs to ensure referral priorities are
met within their team, and ensure the service is responsive to referrals from
multiple sites and agencies
To work with the Community Therapy Lead and Locality Matrons
in developing the strategic and operational management of the Crisis Service
provision.
To work within established Service / Trust policies and
procedures and regularly participate in working parties developing and
implementing policy changes which will impact on all service users.
To represent Community Therapy Services externally on a
locality basis regarding therapy services provided to patients within community
settings.
To work in conjunction with other Clinical leads
to manage Crisis service provision across Co. Durham.
