Job summary

Band 6 Podiatrist two part role - 22.5 hours Community Podiatry working within high risk / tissue viability within Durham and Chester-Le-Street Locality and 15 hours in-patient frailty within University Hospital North Durham.

Main duties of the job

Take a clinical specialist role across within:

o Wound Care / Tissue Viability

Provide leadership within the Team and act as a senior clinician, leading by example.

Provides specialist podiatry advice both within and externally to the Trust

Assesses and treats patients within the team caseload of patients with specialised care needs which includes adults and paediatrics, self-referrals from patients and referrals from other practitioners and disciplines

Participate in the training of other staff and students

Participate in service transformation activity with a view to continuous service improvement and value for money

We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.

Job responsibilities

To be responsible for the assessment, diagnosis, planning, implementation and evaluation of podiatric care for patients within a team caseload with specialised care needs

To provide appropriate podiatric health education to patients and carers

To act as a senior clinician providing specialist advice to and accepting referrals from both podiatrists within the department and clinicians in other fields

To ensure that all interventions are designed using, where possible, evidence based practice and to evaluate outcomes

To work across acute, community and primary health care sectors as part of a multidisciplinary team with direct input to patient care, often highly complex individual cases, and acting as a source of advice to medical colleagues, other health care professionals, patients and carers

To be responsible for the maintenance of appropriate documentation for patient records, reports, audits and Departmental standard.

Leader in Clinical Practice:

Lead by action and inspire others acting as role model, demonstrating the leadership behaviours and values identified by the Trust

Support the Team, helping to ensure that high quality, evidence based podiatric care is delivered

Demonstrate a flexible approach within their clinical practice, helping to meet demands placed upon the service and ensure that waiting time targets and maximum patient choice are delivered

Undertakes reflective clinical practice monitoring personal standards of care and helps to undertake audits in support of the clinical governance framework

Play an active part in the development of local policies and guidelines by commenting on policy changes within their own area

Manager of Resources:

Participate in service transformation activity with a view to continuous service improvement and value for money

Ordering and receipting stock

Professional Development:

Participate in CPD through various means including; peer review, clinical supervision, appraisal etc., where appropriate disseminating information / learning throughout the Department.

Take an active part in student placements / training of students, acting as a mentor where appropriate
