Advanced Nurse Practitioner

Job summary

The Advanced Nurse Practitioner will be experienced in the management of patients at CDDFT with Acute Kidney Injury supporting prompt recognition and treatment to prevent life threatening complications, expedite discharge and ensure appropriate follow up.

To use expert knowledge and advanced clinical reasoning skills to provide a specialist support and advice to the Clinical teams for patients at risk of, or a confirmed AKI.

Working in conjunction with the parent team, the ANP will assess, diagnose, treat and discharge adult patients ensuring appropriate advice, health education and referral to appropriate services.

To perform autonomous diagnostic and prognostic investigations such as renal uss or further imaging.

To make autonomous decisions regarding referring patients for renal consultation using advanced clinical knowledge to underpin decisions.

They will work independently, with a high degree of autonomy, and take part in clinical supervision from a named consultant.

They will be able to manage their own caseload in response to unpredictable patterns of work. They will accept referrals from other health care professionals and work unsupervised.

Assist in the development of the AKI Specialist Nursing Service at CDDFT

Responsible for the ongoing assessment, management of patients with Acute Kidney Injury.

Attend departmental and care group meetings as appropriate to ensure an awareness of developments within area of practice.

Ensure effective communication with nursing and medical colleagues

Work closely with the clinical teams to ensure that the performance standards for are met.

Assist in the development of a robust education programme for staff and patients at CDDFT.
