Job summary

We are currently seeking a Community Sister to join our Darlington Community Nursing Team. The successful applicant will work across the Darlington Locality locality and the surrounding areas from an office base in Darlington. The team cover 8am until 9pm 7 days a week on a duty roster.The post holder will be working in the unplanned team, providing a high level of care and assessment to patients requiring UCR. The role involves providing holistic nursing care to adult patients in their own home or residential care setting. Successful applicants will be required to hold a first level nursing registration with the NMC, additional training specific to the role can be provided.

Main duties of the job

To support coordination and delivery of high quality nursing care that, where possible, meets identified patient need, supports the aims and objectives of the integrated health and social care team, and remains within the boundaries of available resource. Predominantly complex clinical care and urgent care response but also supporting the wider team with planned care when required due to clinical demand. Provides supervision and line management to Band 5 staff nurses, Band 4 nursing associates and Band 3 HCA's.

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.
