We are currently seeking a Community Sister Nurse to Join our Durham city Westl Community
Nursing Team. The successful applicant will work across Durham Area and
surrounding villages from an office base in Meadowfield Durham.The team cover 8am
until 9pm 7 days a week on a duty roster. The role involves providing holistic
nursing care to adult patients in their own home or residential care setting.
The community sister will alos lead a team on nurses and HCA.Sucessful
applicants will be required to hold a first level nursing registration with the
NMC, additional training specific to the role can be provided.
Main duties of the job
To support coordination and delivery of high
quality nursing care that, where possible, meets identified patient need,
supports the aims and objectives of the integrated health and social care team,
and remains within the boundaries of available resource. Predominantly complex clinical care and
caseload management with some coordination and supervisory responsibilities for
health care staff between Bands 2 to Band 5.
The Community Nursing Team provide both planned and reactive care
including Urgent Community Response.

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.
