Job summary
To work in conjunction with the ophthalmology multi-disciplinary team and key stakeholders to ensure that disease pathways and patient flow is continuously monitored against the requirements outlined with the national guidance.
Responsible for the input and production of data to ensure patient treatments are timely and within required standards.
Coordinate cancellations/DNAs through direct contact with patients to reduce the number of missed attendances and prevent lost to follow-up, ensuring that patient vison is maintained through regular attendance.
Provide failsafe systems to ensure timely treatment and continually monitor the treatment pathway.
Main duties of the job
Oversee the patient appointment system, and coordinate treatment pathways
Work as part of the multidisciplinary team to ensure the smooth running of the service, ensuring a quality patient journey within national guidelines
Responsible for ensuring patient appointments are timely and in line with Diabetes service framework guidelines
Provide vital links between treatments, rehabilitation Low Vision Aids (LVA) support for better integration of care by liaising with clinicians and managers to ensure support is given to patients
Attend to patients who are in the diabetic service; to ensure patients are contacted regarding their appointments and that all system/spreadsheets are kept up to date
To support new and/or less experienced staff, as required
Supervise work experience students and apprentices requiring experience of office work
To provide cover as required during absence of colleagues
Ensure that the confidentiality of patient information is adhered to at all times to ensure compliance to Trust and National policy.
Ensure all Trust and department policies and procedures adhered to at all times.
Promote positive customer care for patients and staff

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.
