Job summary
An exciting opportunity has arisen
to work as a Patient Safety Team within a dynamic and progressing Patient Safety
team on a fixed term basis.
The post holder will be
responsible for overseeing the delivery of the Trust's Patient Safety Incident
Response plan. The post holder will provide leadership and support to the
patient safety team. The post holder will also be responsible for coordinating
thematic reviews, patient safety investigations and learning from safety
events.
The post holder will be highly skilled in the
development of patient safety investigation reports and will be a key driver of
Trust wide safety initiatives.
The post holder will be proative
in delivering patient safety improvement and work collabroatively with the
wider trust and external partners.
Main duties of the job
The successful candidate will lead the day to day delivery
of the Trust's patient safety incident response plan to support the delivery of
safer systems of care.
A large part of the role will be ensuring that learning and
improvements occur from patient safety incidents with the aim of reducing
future risk and helping make care safer, whilst ensuring that patients, their
families, and staff are supported.
The post holder will provide line management and
leadership to the paitent safety team and provide patient safety expertise to
the wider team.

We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.

Provide
patient safety expertise/leadership within the organisation; demonstrating
compassionate leadership, visibility and supporting the continued development
of the patient safety culture.
The post
holder will lead, enable and facilitate change within the patient safety arena
to deliver innovative ways of working that are patient safety centered.
The post
holder will manage the the patient safety budget.
Responsible
for ensuring that there are organisational effective systems and processes are
in place for the management of incidents and the use of the Ulysses system, ensuring a sound system exists in line
with national guidance.
As a
senior manager in the patient safety team, ensure ongoing monitoring against
all required standards and approprately escalate any concerns to the management
team, where a service falls below those standards, ensure the production of a
comprehensive action plan, and ongoing assurance of delivery of the required
action plan, to help eliminate poor quality care and improve service delivery
and compliance.
Oversee
the implementation of Patient Safety Incident Response Plan.
Identify
and provide clear rationale for locally defined incidents requiring local
Patient Safety Incident Investigation (PSII) approval.
Complete
training needs analysis to enable the
delivery of the response plan.
Work
autonomously as an investigator and in conjunction with subject matter experts
and clinical colleagues.
Responsible
for the management of day to day overview of investigations.
Manage a
complex workload to tight deadlines and meet the requirements of a large number
of stakeholders with minimal supervision.
Provide
management and leadership to the Patient Safety team.
Work in
collaboration and partnership with other key stakeholders to coordinate events
around patient safety priorities including falls, pressure damage and
infection, prevention and control.
Use highly
developed motivational skills to encourage collaborative working.
Will gather
and analyse a variety of data in different formats from a number of sources to
identify patient safety related trends and themes and prioritise them for
targeted action.
