We
are currently seeking a Team lead to join
our Community Nursing Team for Durham Locality
based in Durham City West at Meadowfield Clinic. The team cover 8am until 9pm 7
days a week on a duty roster. The role involves leading a team to provide holistic nursing care to adult
patients in their own home or care home settings and leading Sucessful applicants will be required to hold
a first level nursing registration with the NMC and have good clinical skills.
Main duties of the job
The
Community nursing team provide both planned and reactive care including Urgent
Community Response. As a Team lead you will directly manage staff aligned to
you, delivering hands on care and supporting the team with training and provide
leadership and clinical over sight in the team.

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.
