Job summary
The post holder will act as an advanced
practitioner and an expert specialist resource in both Parkinsons disease and
related Neurological Movement Disorders. This will encompass the nursing care
and management of people with these conditions, working closely with patients,
carers, Consultants and General Practitioners, therapists and social services.
The working environment will be in primary and secondary care settings across
the trust.
Main duties of the job
The post holder will work
as a senior member of the medical and multidisciplinary team, providing highly
sensitive information, advice, care management and support regarding patients
care and treatment within the hospital and community.
They both promote and
initiate research based nursing and medical practice and influence local
service provision based on measurable results.
Recognise, prioritise and
triage the management of symptoms and implement comprehensive knowledge into
practice. They will question and review practice and respond innovatively and
act as a catalyst to enable new ways of working.
Identify and address
shortfalls in service provision Trust wide, taking a strategic overview of the
service and be accountable for recommending re-designs to the service.
Initiate new specialist
support developments within the hospital and community settings Trust wide and
develop and integrate documentation that facilitates and improves assessment.
Be aware of the need to
network across the speciality of Parkinsons disease and related neurological
movement disorders, both locally and nationally
Work in partnership to
deliver high quality care for those with Parkinsons disease.

To advise and support
health professionals in medicine management including pre and post surgery,
management and treatment for patients with neuro psychiatric symptoms.
To take direct
responsibility in recognising when end of life care is appropriate and support
patient, family, carers and colleagues through this process.
To be responsible for
managing own caseload, including in-patients, out patients and home visits and
provide support and advice to junior colleagues regarding their caseload.
To work as lead nurse for
implementation of attend anywhere clinics and run this service independently.
Provide independent
nurse-led clinics Trust wide and be individually accountable for the patients
assessment and treatment.
To supervise Band 6 nurses.
Provide comprehensive
expert advice for GPs on all aspects of disease management and medication
changes.
To take direct
responsibility for reviewing patients response to treatment by regular
contact, monitoring and detection of side effects, titration and reduction of
medication using expert knowledge and evidence based practice.
Provide highly sensitive
information, support and advice for patients with Sub-thalamic Deep Brain
