An exciting opportunity has
arisen for a CNS within the North Team. we are seeking to employ a dynamic
individual to establish and maintain nurse led clinics for patient with
haematology conditions.
The sussessfull candidiate
will ensure the provision of specialist/expert knowledge and research based
practice to all patients requiring specialist haematology care and provide
senior clinical support for consultant haematologists
You would be required to be
able to Ppan a systematic approach to assess management of patient care in
consultation with the MDT, primary care and other organisations to address the
individual needs of and involving patients and their carers in disease
management, whilst encouraging patient participation in care and service
development
There
is also opportunities to participate/facilitate research projects and initiate
and participate clinical audit of the service in order to demonstrate
clinically effective practice
Main duties of the job
The role of the nurse specialist in haematology is
multifaceted, incorporating key functions as policy and standard development,
education, research and professional development.
Provide direct clinical support for consultant
haematologists.
Provide clinical expertise to primary/secondary care
staff regarding the care of patients in this specialist group, acting as a
resource for health care professionals, patients and carers.
Provide expert advice to specialist cancer nursing group
within the Trust and participate in Regional Haematology Nursing Forums.
Facilitate, coordinate and monitor patients receiving
chemotherapy, radiotherapy and best supportive care for haematological disease.
To establish and maintain a nurse
led service for all haematology patients.

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.

Plan specialist nursing provision for continuity of care,
referring and admitting patients as necessary
To prepare appropriate written information consistent
with best practice and Trust guidelines to enhance understanding
To act as the key worker for all haematology patients
ensuring that all patients and carers are well informed about their disease and
its management providing appropriate and timely information
To communicate diagnosis and treatment options for

To provide telephone access for patients and carers to
allow ongoing support and rapid access to all members of the haematology team
Lead for patient information within the specialty, acting
