offer an exciting opportunity for a suitably qualified and experienced Nurse to
work within the Specialist Coronary Heart Disease & Heart Failure Team
based at the Greenhouse in Annfield Plain. The post covers core hours of
09:00-17:00 Monday-Friday with currently no weekend working although that may
change in the future.
Main duties of the job
The successful applicant will be primarily caring for
patients with coronary heart disease (CHD) referred into the service following
acute events eg myocardial infarction and planned procedures eg elective PCI.
They will also be expected to support the Heart Failure Specialist Nurses
reviewing and caring for heart failure (HF) patients.
They will be directly involved in delivering cardiac
rehabilitation, to both CHD and HF patients, alongside the exercise
facilitators.
There will be an opportunity for professional development
to, if necessary, gain clinical skills
qualification and NMP.
Care is delivered in the patients own home and
in clinic / community venues around Easington

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.
