Job summary

An exciting opportunity has arisen for a staff nurse to join our established Anticoagulation Service based at Bishop Auckland General Hospital. The Anticoagulant Team provide a commissioned Warfarin monitoring and dosing service within the locality of County Durham & Darlington NHS Foundation Trust. We are a small, well established team and the post-holder will have the ability to work autonomously and as part of a team.

Main duties of the job

The post holder will work within the Anticoagulant Team providing a commissioned Warfarin monitoring and dosing service within the locality of County Durham & Darlington NHS Foundation Trust. The role involves ensuring that patients treatment remains therapeutic, drug dosages adjusted appropriately and appropriate interventions are made in a timely manner. The post holder must be able to work autonomously and as part of the wider multi-disciplinary team to ensure safe and effective care is delivered to the patient. The post holder will be involved in managing patients within the clinic setting, dealing with telephone enquiries from patients, carers, GPs and other health professionals. The post holder will provide support to patients who are self-testing and facilitate their independence and self-management of long term conditions.

The post holder will be involved in the monitoring and management of patients County-wide and cover out-reach clinics across Trust sites as required. A car and a full driving license is essential to the role as it will involve the management of patients in out-reach clinics across all County Durham & Darlington NHS Foundation Trust sites.

We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.
