Job summary
The Diabetes Team is looking to recruit a self-motivated and
enthusiastic individual to undertake the role of Health Navigator. The post
holder will be responsible for providing health care systems navigation for
patients who seek help to manage their diabetes. The Health Navigator will also manage a small
team to deliver excellent patient care, link with multi-disciplinary
professionals and co-ordinate the provision of structured education throughout
Co Durham and Darlington.
We are looking
someone who is highly professional proactive, well organised and has a strong
sense of patient care.
This role is on a job share basis
Main duties of the job
Work to provide high quality, person centred structured
education; develop, co-ordinate, deliver and evaluate education provison for
patients with Type 2 Diabetes. Pro-actively work with partner organisations to
develop services, develop innovative projects to suppport diabetes
service. Manage a team of staff
dedicated to deliver structured education, plus support diabetes model through
management of diabetes co-ordinators.
Look for opportunites and develop ways to work with a focus on reducing
health inequalities. To be aware of behavioural change models underpinning service
delivery and be able to deliver education courses along with brief
interventions and presentations where necessary. Strong report writing skills needed to
provide regular reports and attend meetings e.g. Diabetes Governance Board plus
work closely with GP Practices to promote uptake of strucutred education.
The post include working to increase attendance
and uptake of diabetes Structured Education through identification of need and
barriers to access. The post holder must
have excellent management skills, be able to work autonomously and under
pressure

We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.
