Job summary
Wellbeing Facilitators work across County Durham to support people to improve their health and wellbeing. If you have a genuine interest in supporting Community Hubs, Community Groups, and individuals (aged 16+) with behaviour change in the local community then this role is what you have been looking for. As well as working with and supporting to develop community hubs and community groups, you will also work with clients on 1:1 basis to ensure they are accessing the right support, at the right time, in the right place, receiving referrals from a wide range of agencies, including PCNs GP practices: pharmacies, wider multi-disciplinary teams, VCSE organisations (list is not exhausitve).

Main duties of the job
The main role of the Wellbeing Facilitator will be to support the Wellbeing Programme Manager and Hub Coordinators with the overall development and implementation of the service. The role will support the local community and individual clients with the strengthening of community and personal resilience. Reducing health and wellbeing inequalities by addressing the wider determinants of health, such as debt, poor housing and physical inactivity, by increasing peoples active involvement with their local communities. The post holder will particularly work with people with long term conditions (including support for mental health), those who are lonely or isolated, or have complex social needs which affect their wellbeing. Essentially the role will be about connecting and enabling people and communities to access the range of support they require.

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.
