East
community Rehabilitation Team are a small and dynamic therapy team providing
community physio and occupational therapy to adults with physical health issues
within the Easington Locality. The team also works collaboratively with the
community nursing services working over a 7 day rota providing same day
response for patients with nursing/ rehab and care needs that require a 2 hour
response to address.
As part of ongoing
development within the team we are looking for another band 4 Associate
Practitioner to join us.
You will have the opportunity to be involved
with service development as part of the wider community therapy services and an
opportunity to develop your clinical and leadership skills within the
supportive environment of the east therapy team.
Main duties of the job
To carry out clinical/rehab
interventions following treatment plans developed by
Physiotherapists/Occupational Therapists/Registered Nurses
To progress treatment interventions and contribute to
patient review using skills and knowledge
To evaluate and adapt interventions within set protocols
to meet the changing needs of patients as they progress with treatment regimes.
To order, fit and demonstrate safe use of assistive
equipment through Medequip. This may include delivery of items.
Participate in the monitoring of standards of care, and
audit of nursing/therapy practice which is research based and supports the
clinical governance framework.
Coordinates clinical work with colleagues demonstrating
effective time management skills and participates in the monitoring and
reviewing of team performance.
Promote an environment where health promotion and
informed patient choice is encouraged and developed to its maximum potential,
within the available resources
To undertake relevant training to enable the provision
of a high quality service.
Support registered staff
with the induction, training and education of students and other staff
as appropriate

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.
