Job summary

County Durham and Darlington Community Services are offering an exciting opportunity for an experienced respiratory nurse specialist, across North Durham, Chester-Le-Street, Derwentside and Easington.

The community respiratory specialist lead nurse role is to co-ordinate expert specialist respiratory care and review for complex respiratory patients within community clinics and patients own homes. To provide all aspects of expert, holistic respiratory care and support including assessment, diagnosis including diagnostic spirometry, medicines management and oxygen assessment to those suffering complex disease. Providing patient safety and clinical effectiveness, through expert evidence based clinical advice.

The role includes managerial responsibility for a team of qualified nurses and associates practitioners.

The successful candidate will be involved in service development, research, service evaluation and audit, whilst acting as an effective role model within the nursing team, and to other professionals within the multidisciplinary team.

Main duties of the job

Clinical management of complex respiratory patients within clinic and home settings from diagnosis,

diagnostic spirometry, oxygen assessment, long term case management, medicines management, to palliative care.

Decision making and problem solving.

Support the team with leadership and management responsibility for a team of registered nurses, associate practitioners, and admin staff.

Educational role including patients and colleagues.

Support service development including guidelines, policies and procedures, evaluation, audit and research, supporting service provision across the organisation.

Identify, negotiate, and lead change.

Assessing and managing risk.

We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.
