Job summary
Ward
33 at Darlington Memorial Hospital is a busy general medical ward caring for 31
patients.
These patients can have a
wide range of medical conditions and can be complex.
The
discharge Coordinator role is to help support the nursing team in ensuring that
all patient discharges and maintained to a high standard and also to assist
with the complexities envolved.
Main duties of the job
Supporting nurisng staff
with all discharges, including very complex ones.
Liaising with all parts of
the MDT such as Doctors, nurses, OT and physio.
Building good relationships
and making referrals to social services and other support teams.
The
role will involve a lot of autonomy and self organisation.

We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.
